I already made Wind Charges, Snowballs, and Ender Pearls work in the offhand. The only issue is that you're holding the wrong item in your main hand, so they won't work. You need to hold something like air (empty hand), a sword, an axe, or a mace (if I remember correctly 😅). Then the items in your offhand should work.
As for the Fishing Rod, I think it's pretty difficult. I'm still looking into whether it's possible, but I think the chance of getting it to work in the offhand is quite low—around 10%.
