1) Can you give some tips on where to find the other islands, please? So far I have a mob farm and have built out in all directions but have only found the little sand one.
2) Do you know if the zombie loot tables have also been altered to provide gravel drops?

The zombies don't drop gravel. It's a bit of an oversight on my part, but you can still activate your nether portal. Go into your world options and turn fire spreading on ( It's off by default to prevent the island from catching fire early on ) Then you can use oak planks and the lava to start a fire near the portal, search "How to light a nether portal with lava and wood" online if you're unsure how to do it.
If you'd like I can create a "Skyblock Mobs drop" addon that you can use.
I guess I'd only read up on later Skyblock maps. So with this one is there anything in the nether other than the initial island? i.e. can I block out to find a nether fortress trade with piglins, etc?

With it you can make spawning platforms and then trade with the piglins.
The gold ingots will have to come from your overworld mob farm though ( Or zombified piglin farms ), a large part of the original Skyblock experience is patience, grinding and knowledge of how Minecraft works.
This style of gameplay might not be enjoyable for some though, so I'm working on another Skyblock map ( Skyblock Recrafted ) that will make the experience a lot more fun and exciting.

I'm very happy with the way it's challenging the player... just didn't want to block out 1000s of blocks when there wasn't any point :)
