It looks like a grey humanoid zombie with a faceless head with half of it being removed revealing a black inside, white inky uniform, this should be the most dangerous entity as it is really fast, can crawl, can jump, and etc..
(also please check my past comment somewhere below it has really good suggestions I think)
1. The Redrooms - We all know what this is, just level 0, but blood red. I have two ideas for an entity here, one is the Lurker, a dark slender being that is very hostile, and another could be the Duller... uh, just look at the wikis on how it's described.
2. The Dark Corriders - Level 0 but the wallpaper and carpets are darker and moldy (Im talking about actual mold, not like the Bacteria and stuff), maybe a smiler would do and it only attacks the player if they are standing in an area with the light level being 0 but really just the darkness is scary and more liminal.
3. (This is third since I kinda feel like it doesn't fit the mod) - The Poolrooms, just a relaxing poolhouse with white tiles and stairs/slab counterparts of the blocks. This should just be a relaxing an safe level but maybe you could also add the dark poolrooms and a new type of fluid called Infested Water and it is deadly because it has something called the Hydrolitis Plague.
I know some of these ideas are bad but whatever -_-
