You need to try it on recent versions of Minecraft and place the pack at the top of other packs if you have them activated.I just tested it on the latest version and it works

i'm on 26.32, and tested the chaos option in worlds with and without other packs (it was the top pack on the list everytime), even so it only plays default hurt sound :/ the other options are working fine tho

How do you receive damage? I noticed there's a bug where the sound isn't heard when the damage is from fire; I'll fix it in the next version.

(I couldn't see your reply on the app for some reason... sorry for the delay)
It's a 13 pro iphone, but I don't think this is the problem; maybe if the mod is not working properly on some IOS devices. And when I realized the chaos mode wasn't working was when I took fall damage. Then I tried being hit by mobs, taking damage from being out of air and food, and none of the custom sounds played.
I'll be testing it again since you updated the pack recently. And even with that minor incovenience, I gotta say exploring a cave in absolute silence and be blasted with a metal pipe sound has been my favorite way to irritate my friends XD
